Anoka County Seeks Nominations for Public Health Awards

Do you know an individual or group that has made a significant contribution to improving public health in the community? Anoka County would like to recognize them with a 2019 Public Health Award. Nominate an individual or group now through Thursday, Jan. 31. Awards will be presented in the following categories: Youth Focused, Individual or Group: Individual(s) age 18 or younger at the time of nomination or an organization that is youth-led; Adult, Individual or Group: Individual(s) age 18 or older at the time of nomination or a group, partnership, community coalition, organization, etc.
Nominations will be evaluated based on: Leadership: nominee has demonstrated leadership abilities through work and service; Community Health Improvement: nominee demonstrates successes in creating solutions to public health problems and has contributed to making improvements in the health status of the community; Collaboration: nominee promotes collaboration and cooperation between individuals, organizations, and agencies; Advocacy: nominee has been involved in promoting healthy behavior and/or protecting the health and safety of county residents.
Awards will be presented at a county board meeting in April, which is Public Health month. Nominations may be submitted by mail, fax, email or online. Visit anokacounty.us/healthawards for more information and nomination forms.
